Financial Aid Checklist

Application for Federal Student Aid (FAFSA) for the 2008-2009 award years. You will need your 2007 tax return in order to fill out this form. You can complete this at
www.fafsa.ed.gov.
Completed Master Promissory Note (MPN). Please go to
www.wachovia.com/education and click on Stafford loans. If you're enrolling in the Doctorate of Chiropractic you will be entering a graduate/professional program. If you're entering the undergraduate program please list your current grade level. Wachovia will notify Parker when this form is complete. Students not choosing Parker as their lender will need to provide a copy of the Master Promissory Note from the lender of their choice.

www.mapping-your-future.org and click on Entrance counseling. Mapping-your-Future will notify Parker when this is complete.
Please review the applications to make sure all of the above has been completed. Once we receive all of your applications we will begin the awarding process.
